Quebec's Workplace, Health and Safety Commission is making a series of recommendations following the death of a window washer.

Andre Laroche died Aug. 18 when he fell 20 storeys from the Blue Cross building at the corner of President Kennedy and Aylmer.

The CSST says proper safety equipment -- including a harness -- was not used.

The commission also blames inadequate supervision and a defective electrical cable.

A fine of $5,000 to $20,000 will be issued against the window washing company, Mon Laveur de Vitres.

A copy of the report will also be sent to window-washing companies across the Montreal region.
